The following must be submitted to the appropriate offices, no later than the DEADLINE for submission date above:

- A Pre-participation Physical Evaulation (PPE) is required once per year, to reflect a medical examination within 365 days prior to the first day. There has been a change in the requirements for submission of the updated PPE. The History Form and the Physical Evaluation Form within the packet do not need to be submitted to the school and should be kept by the healthcare provider who completes the PPE.
- Only the completed Medical Eligibility Form must be turned in to the School Nurse or Athletic Office no later than noted deadline. Any missing information will result in the delay of your child's medical clearance and will be returned to the parent for proper completion.
REMINDER: ALL MEDICAL ELIGIBILITY FORMS COMPLETED BY A PRIVATE PHYSICIAN MUST BE SENT TO & APPROVED BY OUR SCHOOL DOCTOR PRIOR TO THE STUDENT-ATHLETE PARTICIPATING IN PRACTICE/TRYOUT.
